Definitions
Business User: a representative of an organisation who completes a registration process via Quescreen.com. A Business User can request a User to complete a Quescreen by logging into the Quescreen portal or by downloading the Quescreen App.
User: an individual completing a Quescreen via Quescreen.com or the Quescreen App
What is Quescreen?
Quescreen is a human health risk screening tool for the workplace environment.
Quescreen can support a Business User in:
Recruitment Processes - Quescreen quickly assesses a person’s suitability on physical and/or human health risk grounds against a Defined Job Role before determining if further information and/or screening is required to complete the assessment.
Health Checks - Suitable for a Business User’s existing employees, Quescreen quickly assesses a person’s suitability on physical health risk grounds against a Defined Job Role and produces a Health Check result for the User. A Business User will not receive a User’s Health Check result.
Return to Work - Quescreen quickly assesses a person’s suitability on physical health risk grounds against a Defined Job Role. The result can assist to inform a Business User of a person’s current health risks and aide workplace discussions in a return to work scenario.
A Quescreen is available for purchase online via Quescreen.com by a Business User for completion by a User. A User can complete a Quescreen on any device by web browser, or by downloading the free Quescreen App from Apple or Android stores.
WHA Screening Pty Ltd trading as Quescreen (we, us, our) is making this Quescreen available to assess human health risk against a defined job role. Quescreen will not provide a medical diagnosis.
After the User completes answering personal health and medical related questions, a Business User will immediately view a User’s human health risk against a defined job role on a human body image. Quescreen uses colour coding to identify the severity of risks. For Health Checks, a Business User will not be able to view a User’s result.
This Privacy Statement sets out how personal information that is collected via Quescreen will be handled in accordance with the Privacy Act (Cth) 1988 or Privacy Act 2020 NZ as applicable, dependent on your location.
What personal information will be collected, and why is it being collected?
Quescreen collects the following personal information about a User:
- mobile phone number – so that the User can be contacted if needed and to authenticate personal details, as required
- name – so that we can personalise the Quescreen result to the User
- email address – so that the User can be contacted if needed to authenticate personal details, as required
- date of birth – to assist Quescreen assess human health risk, and so the User can be verified by us in future
- postcode – so that the User can be verified by us in future
- Current or proposed job title – to select the appropriate risk profile template and complete the appropriate Quescreen
- Medical and health history and information – by answering a series of questions we can assess a User’s human health risk against a defined job role.
We will also ask if a User is 18 years of age or older. If a User is under 18 years of age, we will ask the User if they have a parent or guardian (parent) present who can consent to the User sharing their results. We will collect the first name and surname of the User’s parent. If a User does not have a parent present at the time of the screen to consent for the User to share their results, the Quescreen cannot be completed.
Quescreen will ask the User to consent to the following:

The User has viewed the User instructions and understands Quescreen is a human health risk screening tool for the workplace environment. The User understands by completing personal, medical and health related information, Quescreen will assess a User’s human health risk against a defined job role, but will not provide a medical diagnosis. The User also understands Quescreen relies on honest responses from the User. The User will complete the Quescreen questions truthfully and as accurately as possible to enable Quescreen to provide an accurate as far as possible assessment.

The User understands that the Business User will have access to view their responses to the Quescreen questions, their personal profile, including their last name/surname, first name, preferred name, middle name (if applicable), date of birth, gender, drivers licence status and category, height, weight, current medications (if applicable), COVID-19 vaccination status and other broad lifestyle and medical history information) and the Quescreen results. The User consents to the Business User accessing the User’s personal information, including their personal profile, medical and health related information and Quescreen result, for the purpose of understanding their physical, medical and/or human health risks against the defined job role. We recommend a User check with the Business User and satisfy themselves regarding the protection the Business User will provide for their information including personal information, medical and health information prior to consenting to the release of their information to the Business User, regardless of whether or not they are employed by the Business User.

Where a Business User purchases a Health Check for a User, the User understands a Business User will not have access to a User’s responses to the Quescreen questions, their personal profile, including their last name/surname, first name, preferred name, middle name (if applicable), date of birth, gender, drivers licence status and category, height, weight, current medications (if applicable), COVID-19 vaccination status and other broad lifestyle and medical history information) and the Quescreen results. A Health Check result is only available to the User.

The User understands any physical and/or health risks identified against the defined job role by Quescreen will be for the User to personally seek further advice and/or treatment. The User also understands WHA Screening Pty Ltd is not engaged to provide the User health care or diagnosis for treatment or other related purpose.

The User acknowledges and confirms the User has read and understood WHA Screening Pty Ltd’s Quescreen Terms and Conditions, WHA’s Privacy Policy and Quescreen Privacy Statement.

The User consents to Workforce Health Assessors Pty Ltd, WHA Partner Facilities (an allied health business in Australia or New Zealand who enter a License Agreement with Workforce Health Assessors Pty Ltd to conduct pre-employment medicals to candidates) engaged by WHA Screening Pty Ltd and/or medical professionals accessing the User’s Quescreen result for the purposes of undertaking a physical Medical Assessment via the following additional service.
Medical Assessment Additional Service
The following service is available for purchase by the Business User where a Medical Assessment is sought relating to the Quescreen result. Fees for this service is available by contacting WHA.
Where a Business User purchases a Medical Assessment, any clarification or further information will be provided to the Business User only. A User will be required to consent in writing to the Medical Assessment and attend an appointment at a WHA facility for the purpose of undertaking a physical Medical Assessment.
Medical Assessment
A physical medical assessment at one of WHA’s facilities across Australia and New Zealand to screen a User for risk factors that may limit their ability to perform a job role safely and effectively.
A medical assessment varies depending on the job role a User is being assessed for and may include:
- Vision test
- Hearing assessment
- Drug and alcohol screen
- Cardiovascular examination
- Musculoskeletal examination
- Respiratory - Spirometry (lung function)
- Functional capacity assessment
- Fitness testing
Quescreen collects the following information from a Business User to register a business account:
- Contact first name / surname
- Position title
- Email address
- Company name (or alternatively partnership, sole trader or trust name)
- Address
- ABN/ACN
- Web address
When a User uses Quescreen
Quescreen will record the User’s personal contact information, Quescreen responses to personal health and medical related questions, the defined job role, date and time the User completed the Quescreen and the User’s results.
If the User chooses to prove their identity through authentication, we will send the User a unique code via email or SMS.
Recruitment or Return to Work Quescreen: based on the User’s Quescreen responses to personal health and medical related questions, Quescreen will assess the User’s human health risk against a defined job role and display risks on a human body image. Quescreen produces a report with the User’s Quescreen responses and human risk profile; accessible to the requesting Business User as detailed in the following paragraph.
Results are only provided to a Business User where a Business User purchases a Quescreen (Recruitment or Return to Work) for that User. Results are only provided to a User where a Business User purchases a Quescreen Health Check for that User.
Health Check: based on the User’s Quescreen responses to personal health and medical related questions, Quescreen will assess the User’s human health risk against a defined job role and display risks on a human body image. Quescreen produces a report with the User’s Quescreen responses and human risk profile; accessible to the User only.
How will personal information be collected?
When a Business User purchases a Quescreen for a User; the Business User can elect to provide Quescreen a User’s email address, first name, surname, date or birth and/or mobile number, to allocate a Quescreen. If a User’s personal details are not known at the time of purchase, the Business User can share a unique access code (generated by Quescreen) with the User at a later date to access Quescreen. Where a User’s personal details have not been provided by the Business User and a User accesses Quescreen using a unique access token, Quescreen will prompt the User to provide their personal details.
Quescreen will collect a User’s:
- first name and surname
- mobile number
- email address
- date of birth
- postcode
- current or proposed job title; and
- personal health and medical related information through completion of a Quescreen.
Upon registering a Business account via Quescreen.com, Quescreen will collect a Business User’s:
- Contact first name and surname
- Position title
- Email address
- Company name, partnership, sole trader or trust name (as applicable)
- Address
- ABN/ACN
- Web address
- Billing details
How will personal information be stored?
Quescreen will retain a User’s personal health and medical related information and results in accordance with our Privacy Policy and applicable privacy laws. This information is not de-identified, however for reporting or analytics, all data identifying an individual is de-identified.
For both web-browser and Quescreen App Users, we will only store a User’s encrypted access token (which is used for authentication) in the web-browser/Quescreen App locally. A User can delete the encrypted access token by logging out of the Quescreen App, cleaning out their web-browser cache or un-installing the Quescreen App. A Users encrypted access token is not stored in our secure server.
A User can access their personal information and responses to a past Quescreen by logging into Quescreen and viewing Quescreen History. If a User downloaded the Quescreen App to complete their Quescreen, a User can log-in to the Quescreen App at a later date to access their personal information as required.
Where a Business User purchases a Quescreen (Recruitment or Return to Work) for a User, the Business User can access that User’s results by logging into the Quescreen portal. Where a Business User purchases a Quescreen Health Check for a User, a Business User will not have access to that User’s Health Check result in the Quescreen portal.
The information is stored on our secure server. The server hosts the website. The server is a cloud-based facility, using infrastructure (cloud-based server) located in Australia.
How will personal information be used and disclosed?
We will use a User’s Quescreen to determine human health risk against a defined job role. Results are displayed on a human body image with colour coding indicating the severity of risk. In the instance of a Recruitment or Return to Work Quescreen, a Business User who purchased a Quescreen for a User can access that User’s results via the Quescreen portal. In the instance of a Health Check Quescreen, a Business User who purchased a Quescreen for a User will not have access to that User’s results via the Quescreen portal, however can view a User’s booking status (informing the Business User if a User’s Health Check is ongoing, incomplete or complete). A User’s Quescreen result may be shared with Workforce Health Assessors Pty Ltd, WHA Partner facilities (an allied health business in Australia or New Zealand who enters a License Agreement with Workforce Health Assessors Pty Ltd to conduct pre-employment medicals to candidates) engaged by Quescreen and/or medical professionals accessing a User’s Quescreen result for the purposes of undertaking a physical Medical Assessment relating to the Quescreen result.
We will receive de-identified analytical data from iTunes and Google Play about use of the Quescreen App including the number of downloads, average use time and deletions.
De-identified analytical data may be shared for statistical purposes with Workforce Health Assessors Pty Ltd including number of Users, gender and the age range of Users.
We will not use or share or disclose a User’s information including personal information for any other purpose.
Can personal information be deleted?
The Quescreen App retains a User’s personal details so that a User can simply and efficiently complete a new Quescreen. A User can un-install the Quescreen App at any time to delete their personal details from the App.
A User can request us to delete their information including personal information held in the secure cloud based server at any time after they complete a Quescreen, via the ‘Contact Us’ request form on the Quescreen website or via the Quescreen App. This means, once we delete the User’s personal information a Business User will no longer have access to the User’s information via the Quescreen portal.
We, however, have no control or responsibility over how the Business User uses the User’s information including any personal information and health information held by them. We recommend a User check with the Business User and satisfy themself regarding the protection the Business User will provide for their information including personal information and health information prior to consenting to the release of their information to that Business User, regardless of whether or not they are employed by that Business User.
Can a User correct or access personal information?
If a User’s personal information changes or a User believes our records are not up-to-date, complete and accurate please contact us. If we agree the information needs correcting, we will take reasonable steps to correct the information.
If instead we do not agree the information needs correcting, a User can ask us to put a statement with their personal record explaining what they say needs to be corrected.
If we refuse to correct a User’s personal information, we will provide a User with a written explanation for that refusal. We will try to resolve requests within 14 days of receipt of a User’s written request or 30 days where the matter is more complicated. We may charge a User a reasonable fee to correct that information.
A User can request a copy of their results via the ‘Contact Us’ page, by contacting the Business User who requested it, or by logging into the Quescreen App (if the User originally used the Quescreen App to complete a Quescreen).
To ensure maximum security of a User’s Quescreen including their personal, health and medical related information recorded in the Quescreen, a User will not be able to directly access any of their information held in the secure server.

A User and Business User can obtain a copy of our privacy policy from the Quescreen website or by contacting us using the contact details set out at the end of this privacy statement.
The privacy policy contains information about how a User or Business User may complain about a breach of:
- the Australian Privacy Principles (APPs)
- the New Zealand Privacy Act Principles
- how we will deal with such a complaint.
